Now I see why I sometimes get inconsistent results.
To be able to use Hoefler Text and Gill Sans (after the Combo update xdvipdfmx no longer crashes) as one of my standard fonts I made copies of the dfont versions from a 10.5.8 system and placed them in my personal Fonts folder (~/Library/Fonts). The system picks this up and all my files using those fonts work ok. To test I removed the dfont versions and tried to test and things still work ok (?) but if I log out and back in the problems are there; the dfont versions were in my personal font cache if I use them and that gets cleared when I log out and back in.
In any case, if you haven't played the game I believe a simple file that uses Hoefler Text and/or Gill Sans in Bold, Italic or Bold-Italic will show the problem if xdvipdfmx doesn't crash with a Segmentation Fault. Of course you need to have updated to OS X 10.6.2.
