\fontspec[Script=Malayalam]{Rachana} (font name can be different, I  
don't have it) should be used ... ('texdoc fontspec' is a very useful  
command). Select a script from an OpenType font will help to create  
all the ligatures and accents. Could be there are some flaws in the  
font itself. Could you repeat your test with other fonts?
