There have been threads on fill-in PDF forms either here or on the OS
X TeX list. I'm not quite sure I'm remembering the outcome correctly,
but if memory serves right the sad conclusion was that, in order to
allow forms to be filled in using free applications (like Adobe
Reader, xpdf or, on OS X, Preview), you needed to use the commercial
Adobe Acrobat. Again, I may be mixing things up, but I'm not: there is
somewhere a bit to activate in the PDF file, authorizing Adobe Reader
to edit the forms, and only Acrobat (the commercial stuff) can
activate this bit.
Hope knowledgable people can confirm or correct the above.
