[XeTeX] Vertical text in margins

This is probably more a latex question...

I want to place vertical cjk in the left margin (only) adjacent to a 
\section{} or \chapter{} in a book. \marginpar etc don't seem to like 
the left margin.

I currently have sample1.pdf and want get closer to sample2.pdf

Can anyone suggest where I should be looking?
