Yes. I have a file with many, many test cases, a quarry to create a
sample from, if something fails, or, by commenting packages etc.,
find an interaction that can cause a failure. Probably not the best
way, but it's easier to remember one file name ...

Using this modified example you gave:

	\documentclass{scrbook}
	\usepackage{fontspec,xunicode}
	\usepackage{xltxtra}
	\usepackage[xetex]{graphics}
	\setromanfont[Mapping=tex-text, ItalicFont={LM Roman Italic}]{LM
Roman Regular}

	\begin{document}
	test \emph{test}\footnote{test}
	\end{document}

I get (without problems in the ruler):

When I use "\setromanfont[Mapping=tex-text]{LM Roman 10pt}" I get
quite the same result. Pdffonts (from xpdf) tells me (your example
first, then my abbreviation):

	name type emb sub uni object ID
	------------------------------------ ------------ --- --- --- ---------
	HUSPRA+LMRoman17-Regular             Type 1C      yes yes no       5  0
	JXEQRA+LMRoman12-Italic              Type 1C      yes yes no       6  0

	name type emb sub uni object ID
	------------------------------------ ------------ --- --- --- ---------
	XGJZMD+LMRoman10-Regular             Type 1C      yes yes no       5  0
	VDXYMD+LMRoman10-Italic              Type 1C      yes yes no       6  0

The picture is:

Can you send me your LOG file, best when you also use Latin Modern? I
think I have the means and the understanding to find a difference
before it's afternoon ...
