If you're losing the 'T' after an opening quote (“), this suggests to 
me that you aren't saving your document as Unicode (UTF-8). You're 
probably saving it using MacRoman encoding, where “ is a single byte, 
but XeTeX reads it as UTF-8 and this byte value is then seen as the 
first byte of a two-byte sequence representing a single Unicode 
character.

You can set the encoding in TeXShop's Preferences dialog.
