[Tuglist] macro package for typesetting questionnaires and survey forms?

Question:

Are there any LaTeX (or Plain TeX) macro packages for typesetting
questionnaires and survey forms?

The basic elements needed are:

	- typesetting of the questions with the ability to leave space 
	  on the printed form for answers, perhaps spanning multiple
	  lines
	- typesetting explanatory text near the questions
	- questions in the form of tables to be filled up
	- nested questions (i.e. questions to be answered only if a 
	  "higher-level" answer was "yes")
	- checkboxes and other ornaments

If I had to start from scratch implementing such a package, where would 
you recommend I start from?
