Van der Laan implemented Knuth's indexing macros from TeXbook p.423 (a `good
approximation to an index') in order to have a real automated index.  With
slight modifications to those macros I added hyperlinks.  Unfortunately I
didn't manage to add them also for the `range notation': I wanted to hyperlink
the first and last page number of the range but too difficult are the macros
involved.  Another lack is the one of sub entries.  Below the test file.  Works
for me.  Thanks to Knuth and van der Laan.
