That code (which was indeed suggested to me by David) has been in use in
several "extension" font packages for musixtex for about 10 years.
musixtex is normally used with plain TeX but is also usable with LaTeX.
The non-definition I encountered was with plain tex.

I'm guessing that something like

\def\DeclareFontSubstitution#1#2#3#4{}

should be added to that code.
