Normally either when you try to use a font that isn't in the
name-database or when luaotfload detect that the cache has been
created with an older version.

Normally you have only one luatex-cache location. You can do
something like \font\test={blub:mode=node;} to check which one
luaotfload is using in your case. Or run luaotfload-tool -u and look
which one it is updated. 


Why are you asking? Do you have some specific problem?
