No, not even in tlcritical which you probably don't use.

Neither.

That is not from TeX Live, but probably Mac or so.

Can you send your texlive.tlpdb (best gzipped?).

And the file generated by the following command
	fmtutil-sys --all 2>&1 > fmtutil.log
