Only on "system call level" as Zdenek put it. At soon as the tex
macro level is involved too (through inputenc, \input,
\includegraphics, writing file names to files for other tools like
bibtex and biber) things can get very complicated and messy. Don't
forget that the main task of a document is to create something
printable  so a large part of (La)TeX concentrates on "how to get
from the input ä the char ä in the current font" and not on "how to
tell my OS what ä means in a file name". 

Actually with lualatex you would have much less problems on the
macro level as it has to do less magic (catcode changes and
reencodings) when accessing chars.
