In ptexlib.h, there are

#ifdef HAVE_CONFIG_H
#include <w2c/config.h>
#endif

and

w2c/config.h includes w2c/c-auto.h.

In my "handwritten" w2c/c-auto.h, there is a line

#define off_t  __int64
