[tex-live] tl2011: ptex format generation error

I have to fix about this problem for pTeX and e-pTeX.
I'll do when I come back to home.
Karl, please give me about 12 hours more.
( I preserve the SJIS default input encoding for win32
  since most users are using SJIS. )
