As I recall, the mktex*-enabling initializations are done in
kpathsea/tex-file.c.  (Calls to init_maketex.)  Can you check your
sources to see if there are changes in that area?

The explicit texmf.cnf settings are fine as a workaround, but they
shouldn't be necessary.
