    Well, this can only be true for `simple' packages.  

It is a fact.  It is how the texmf-dist tpm's have been updated ever
since Fabrice brought texmf-dist into existence.

Right.  Six tpm's = six TL packages.  I see nothing inconsistent with
what I said.  Maybe humans think of it as one CJK "package", but the
only files in CJK.tpm are the ones in subdirs named CJK.  There is no
benefit to be gained by trying to do anything different, as far as I can
see.

texmf is a different story, not rigorously based on subdir names.

    I can't see any solution to get an automatic update.  

What do you mean?  It is already done, and has been for a couple of years.

    How can those special cases be identified?

Looking at the code in Tpm.pm et al.
