Just answering this point: pdftex.cfg is no longer searched using
TEXINPUTS, and has its own search path:
  PDFTEXCONFIG = .;$TEXMF/pdftex/{$progname,}//
which no longer depends on the right something.$progname setting in
texmf.cnf.
