Thanks, that listed the textwidth as 426 pts (150mm) which is fine. But
I've narrowed the problem a little, its in the driver: 

dvips works perfectly:

   \usepackage[dvips,a4paper,body={150mm,230mm}]{geometry}

but pdftex gives the 140mm wide textwidth pages - they are scaled

   \usepackage[pdftex,a4paper,body={150mm,230mm}]{geometry}

Which narrows it down a little.

This is a fresh TeX-Live install on a vanilla RedHat 9.0
box with no post install modifications.
