For the record, I've just installed your (revised) fixes for pdftex in
pdftex r784 and TL r45936. I added most of the explanation you've sent
as commentary in pdftex.web, but didn't change any of your code.

The three simple tests I wrote for it are at

I submitted your (HK) fix for xetex at
https://sourceforge.net/p/xetex/bugs/143.
