[tex-hyphen] naming Swiss German hyphenation patterns

That's your package after all (I guess that you are going to ship it
together with the rest of German patterns?), so naming is probably
left to you?

But we or you can add one version (hyph-de-ch-19xx.tex) of both files
to hyph-utf8 as well (at least until someone comes up with a general
scheme what to do with different versions of patterns, let's stick to
a single version for some time).
