[tex-eplain] Hyperlinks and index.

I am contemplating a macro pair which will allow an automatic 
generation of hyperlinks from the index and to the page listed. One 
macro would establish an anchor on  each page and the other would 
create the linkage by working on each line of the book.ind file either 
outside the importation of book.ind or through a separate program that 
would add hyperlinks to book.ind.

Has this been done before? I don't want to reinvent the wheel.
