[tex-eplain] Minor indexing gotcha.

The command
makeindex book.idx
produces a file book.ind
But the command
makeindex book.jdx
also produces a file book.ind
so one must use
makeindex book.jdx -o book.jnd
if one is generating multiple indexes from a single file. 

Eplain knows about multiple indexes. Makeindex does not.
