MakeIndex chokes on page numbers prefixed with C and D, because it
takes them for roman numerals.
BTW, I would not assume that makeindex could not be configured to get
around this, unless you have actually checked. There are a lot of
configuration possibilities in that program. (Not that I know about
this particular case one way or the other.)
