[tex-eplain] Using eplain indexing with Context

I suspect it'd be easier to deal with those redefinitions (ie, hacking
eplain.tex to, say, \eplainfootnote instead of \footnote, etc.), than
trying to "whittle down" Eplain to indexing only, for precisely the
reasons you have encountered.
