[tex-eplain] Typesetting index.

In some technical books it is a strategy to print the page numbers in 
different faces:  e.g., regular items in roman,  definition of the term in 
bold and examples of use in italics. _The TeXBook_  uses a similar scheme. I 
can of course edit the .ind file after the fact to change the typefaces but 
it would be more convenient to do it at the point of creation of an \idx or 
\sidx tag.

What is the best method for this purpose?
