[tex-eplain] a new index bug?

That's for sure.  In different incarnations.  I think every maintainer
of Eplain has had a crack at it :).  I know I myself have tried to fix
it more than once, but a counterexample always appears.

If anyone can provide a sample document that fails with the current
source, I expect that would be helpful to Oleg ...
