looks like that lm.map is installed. Run

updmap-sys


then look if the lm fonts are found with

kpsewhich t1lmr.fd

if not, then the lm fonts are not installed.
You could also try to use
\usepackage{mathptmx} instead of lmodern.

If all doesn't help, then your TeX-installation must be buggy
