[pstricks] pst-dbicons Questions

   I'm trying to install pst-dbicons by following Herbert's instructions in
the README file:

   Running the Makefile halts because it cannot find the .ins file (which
exists).

   Following the alternative approach

latex pst-dbicons.ins
latex pst-dbicons.dtx
makeindex -s gind.ist pst-dbicons
makeindex -s gglo.ist -o pst-dbicons.gls pst-dbicons.glo
latex pst-dbicons.dtx
dvips pst-dbicons.dvi
ps2pdf pst-dbicons.ps

works for the first latex run, but bombs on the first makeindex:

[rshepard at salmo ~/data/LaTeX/pst-dbicons]$ makeindex -s gind.ist pst-dbicons
Couldn't find input index file pst-dbicons nor pst-dbicons.idx.
Usage: makeindex [-ilqrcgLT] [-s sty] [-o ind] [-t log] [-p num] [idx0 idx1 ...]

   There is no .idx file.

   What have I missed?
