the dvi is okay,
compare the version of your pstricks-add.pro
Should be

% Version 0.14, 2007/09/17


otherwise load it from http://perce.de/LaTeX/pstricks-add/
and copy it into your texmf tree. You'll find the place with

kpsewhich pstricks-add.pro
