Rich, did you tried the LaTeX example or did you
import it into LyX? However, send your source and
the logfile.

maybe that your VERSIONS file is too old, but the files are rather
out of date. Use always \listfiles as first line in your docs
and then look into the logfile for the file list. But old files
cannot be the reason, because there is nothing new in the est file.
