[pdftex] Multiple references to image fails.

It isn't obvious to me that plain (pdf)(e)tex should produce smaller files.
I'm more used to worrying about the total number of pages that will
get printed than the size of files.  Are there realistic examples where
(pdf)LaTeX or ConTeXt produce inflated pdf files?  Does using miniltx
and graphix, etc. increase the size of .pdf files created with plain?
