[pdftex] Creating multifile collection with clickable links

Using hyperref one can create a document with clickable links:
 \ref-->\label, \cite-->\bibitem -\cite etc

Table of contents and index are also clickable.

One can create clickable cross references using xr-hyper.

The question is how one can create collection so that common "table of
contents", "index", "references" would be separate documents but there
will be still links

toc --> corresponding page of the collection,
index --> corresponding part of the collection,
\cite-->\bibitem

Just to clarify: I want to have

common.pdf (toc, index, bibliography)

volume1.pdf
volume2.pdf
volume3.pdf

and that toc in common.pdf was clickable with the link to
corresponding chapters, sections etc in volume1.pdf,...
also index in common.pdf was clickable with the link to corresponding
pages in volume1.pdf,...

and \cite{} in volume1.pdf, .... were clickable with links to the
corresponding bibitems in common.pdf
