[pdftex] how to concatenate PDFs with outlines and hyperlinks?

"The patch has been tested with PDFs created with pdfTeX, Version
3.14159-1.10b (Web2C 7.4.5) and hyperref.sty 2003/01/22 v6.73n."

Please update pdftex and hyperref.
