[pdftex] how to concatenate PDFs with outlines and hyperlinks?

Dear All,

Since I have given up trying dozens of unworkable solutions,
I have prepared a patch to Ghostscript 8.54, which will make
hyperlinks and outlines come out right with PDFs concatenated
with Ghostscript. Get more info form:
