Very old version.

On FC5 I get:
$ /usr/bin/pdftex --version
pdfeTeX 3.141592-1.21a-2.2 (Web2C 7.5.4)

I haven't had a chance to look at your example, but I strongly
recommend getting a current version of pdftex.  Version 1.30 is
available from TeX Live and should work on most linux systems, even
old versions.
