[pdftex] Updating pdfetex etc.

Thanks for the useful information in re: Context. But I am
working in both Context and plain pdf(e)tex. In the pdftex
manual it spells out that the default paper sizes are now set
within the compiled product and not referenced from an external
file. I tried to RTFM and follow the instructions but I failed.
Please see pages 5 and 6 of the latest version of "The PdfTeX
user manual". I am trying to follow those instructions, and
unfortunately not succeeding.

I can of course set papersizes explicitly in every instance
using the primitives. But I would rather set and forget. I am
in the USA and I use 8.5 x 11 paper.
