[pdftex] pdfform

I saw yesterday in asXiv tex file which has inside two document environment.
I think it was prepared using TeX. I tried to do the same using LaTeX,
however application used only first document environment. I have little more
general questions.

1. How using hyperef package I can design form in PDF file?

2. If I can prepare form in PDF file is it possible put on this 2 radio
buttons such that selecting any of them allows to user read different
documents?
