/MediaBox is already set by pdfTeX, now you have two /MediaBox with
different contents. Thus the behaviour is undefined.
Use either a setting as portrait A4 paper with rotated contents
(package pdflscape) or choose a landscape A4 media format without
rotation.

I cannot find the key /PageSize in the PDF specification. Therefore
I would drop this key value pair.
