afaik indeed a node is added (this is why etex is not guaranteed tex-compatible 
when lr typesetting is enabled)

This is a general problem with r-l typesetting, the same happens with specials 
and literal pdf (think of color); afaik neither etex, nor omega take this into 
account, so it's left to the macro packages to take care of it
