[pdftex] TOC-like hyperlinks inside/to index (Marc Lazareff)

Greetings Marc,

I recently had a similar question and received great encouragement from this
list.  Our index is in a volume almost completely of its own, so at the
beginning of it I now define \lhead:

\bf{%
\href{file:indexvolume\#page.Symbols}{Symbols}\ %
\href{file:indexvolume\#page.A}{A}\ %
\href{file:indexvolume\#page.B}{B}\ %
