[pdftex] Menu with Icons at top of page

Hello All,

How can I add an icon to a button in a form,
I have tried the following but it does not work, does anyone have any ideas.
What I am trying to do is put a menu at the top of my document with icons
for back forward, search.

I am thinking of using the package fancyheader, but I am having some basic
problems.
