[pdftex] TeX capacity exceeded, sorry [number of destination names=20000]

I've met this frequently with (auto-generated) large manuals.  A 1000-page
manual can easily have 20000 destinations (one I am thinking of has 3000
in the index alone).  However, with a tolerably recent pdftex the settings
mentioned in the original post do solve the problems. I think the problem
may be the `tolerably recent' part, and I recall a user of my makefiles
with (I think) pdftex-0.13d could not overcome the limits.
