[pdftex] Italics and Bold

This is not a pdf*TeX question, but a LaTeX question -- or, more
precisely, a LaTeX(2e) question.  For the past 10 or so years, you've
been encouraged to use \textit{} and \textbf{}, as any recent book on
LaTeX will tell you.

\usepackage{bm} or \usepackage{amsbsy} or use Lucida fonts.
