[pdftex] common header for latex files

Hi all,

I'm a new user and I would be very glad to have some suggestions in order to
create .dvi (latex), .ps (dvips), .pdf (pdflatex) and .html (latex2html)
documents using one common header for my .tex files, knowing also the best
image format (.eps or .png or other).

My question relates to pdfs created for on line
viewing using Acrobat Reader.

There are ways to make a pdf appear full screen and
without the usual navigation aids etc. using either
pdftex or Context. However one can maneuver out of
full-screen mode with an alt-esc key combination.

My question is: can one trap the alt-esc in some
way so that the key combination would no longer
restore the navigation aids, thumbnail and bookmark
tabs and so on. Of course an exit button would be
needed on every page.
