[pdftex] pdflatex bookmark problem

Very very old, current is 14h.

A current one is available at
  http://www.tug.org/applications/pdftex/pdftex.def

Upgrade to v1.3 2000/09/19. The versions before contain
bugs and can be the cause of your problems.

It is better to load ifthen before hyperref to 
give hyperref a chance to detect the package.
