[pdftex] Win32 pdftex with Tom Kacvinsky patch

Of course, I had rebuilt the format file. However,
I put it in
C:\fptex\texmf-local\web2c
without realizing that there was a pdftex.fmt in   
C:\fptex\texmf-var\web2c
which got found first.
If I'd got the familiar "fatal format error, I'm stymied"
message I would have known what was up.

Anyway, it's OK now.
