[pdftex] Possible bug in pdftexinfo.tex

You'll need to run it more than once and call texindex on index files
(and to work out which ones are relevant), if you have a moderately
complicated .texi file.   When one is used to texi2dvi, one does not
even know what the `other issues' are....
