That's what eTeX (or pdfeTeX) is good for:

\left. A \middle{/} B \right.

The primitive \middle is new to eTeX and not available for TeX proper. Its 
effect can be simulated using the command \biggm, \Bigm, or \Biggm (each 
selecting another fixed size).
