[OS X TeX] Index formatting question

This is a TeX  (Makeindex?) question rather than TS.

If I generate several index entries, one indicating a page range, e.g.
\index{determinant!2@\protect$2\times2$|(}
\index{determinant!2@\protect$2\times2$|)}
and others with the same heading, indicating specific pages, e.g.
{determinant!2@\protect$2\times2$!additivity}
{determinant!2@\protect$2\times2$!homogeneity}
they print as entirely separate (sub)entries.  Is there a way to get the output in the form

determinant
$2\times2$, <page range>
additivity, <page>
homogeneity,<page>

instead of what I get, which is

determinant
$2\times2$,
additivity, <page>
homogeneity, <page>
$2\times2$, <page range>

???
