[OS X TeX] tex live utility no longer working

You should install MacTeX instead - http://www.tug.org/mactex/index.html

This is Apple being annoying. In System Prefs, under Security & Privacy, there is an option at the bottom of the General tab. I set this to "Anywhere", but Apple advises against this for security reasons.
